It's a tool to drill a hole at the intersection of the crank snout and balancer pulley. You drill and ream a hole there and then drive a round pin in there to keep the balancer from ever spinning on the shaft. If you don't tighten the pulley bolt enough you can count on eventually spinning the pulley on the shaft if you don't pin it.
