    i hate to disagree, but on my FLT lvl3, i blew both the input and output shafts at different times.. both times i had park, with the output shaft, i had reverse.

    That's because the shaft must have been broken at an angle and still tied it up.

    What is the OP's cooler flow? Most likely the previous converter failed and filled the cooler with debris. This would account for a quick second fail and no oil from a line.

    I checked the input shaft while the tranny was out. This morning I checked to see if it had cooler flow. I pulled the line am had good psi. So I tried to move the truck. It backed up and forward.... I put it back in reverse and had nothing. After it wouldn't move I checked the cooler line. Had no fluid. It hasn't had fluid flow or moved since. I did flush the cooler lines but not the tranny. I did clean what very little trash was in the pan and do a new filter. That's all though. Should I send the stall back to c-d to see if it did break the first time?

    Did you ever figure out what it was? I am having the exact same issue.... was wondering if it was a stuck valve on the VB or...?
