If you want to keep the disks in back you'll need the 4 wheel disk master cyl and propotioning valve. Maybe the 99-04 half ton stuff would work. As for the wheels... have you thought about converting the front to 8 lug rather thatn the back to 6 lug? I don't know of a cheap and easy way to put 6 lug wheels on an 8 lug truck but you can buy adapters to put 8 lug wheels on a six lug axle.
