Hurst has a truck megashifter that has a console/box it mounts in. Personally I would put a ratchet shifter with a reverse lockout in there but that's just me.

ive always wanted to do a ratchet shifter in my truck. my buddy has one in his 4x4 blazer which is badass. i just love the way it feels
if you cant find a ratchet that you like, you can always configure a setup for paddle shiftersthat would be sweet
