    Might be valve seals maybe

+1 Sounds pretty typical valve seal issue on start up.

With the smoke does it have a "metallic" weird taste? Do you have a test light or noir lights to make sure injectors are pulsing and not getting constant signal. Possible injector stuck open? When I was working out the base idle tune on my megasquirted turbo neon had similar issues but weird that you're truck was fine and now acting up. Leads me to mechanical. On you single low cylinder pull plug see if there is oil again also drop little bit of oil into cycle and do a compression check. If pressure comes up quite a bit prob a ring problem. Just throwing out possibilities.

We can't find anything in the tune but on start up it is pig rich, 10 on the wide band.

Hmm mine is funky on startup but prolly bc my roommate and I don't know know how to idle tune. It running rich would explain the rough startup but idk about the smoke. Is it blue smoking on startup or just black smoke? Could also be bc you have a dead cylinder but compression wasn't too bad. You put new plugs in?

New plugs, I'm thinking its fuel probs. Its black smoke so bad its killing the shit out of my grass every time I park it somewhere and start it up lol. It surely can't be tune related since it was fine before..... ?

I'm gonna check injectors tonight. I'll make sure they're pulsing and probably go ahead and pull them out and clean them . I did notice when we had the comp hooked up to it that it took a while for the o2 sensors to warm up and start reading properly.

Check coolant and air temp sensors make sure they are reading properly. Computer could be putting more fuel in if it thinks ambient is -20 below... Should only take the ecm a few moments to go into closed loop.
