Did my motor bite the dust?

Ok, So I drive to work yesterday morning and no problems. I was running late for a meeting so I park in the closest parking lot. After the meeting I go to move my truck to the far parking lot where I am supposed to park and I have a hard time starting my truck. It doesn't start and spits or sneezes through the intake and I lose all power. I pop the hood and find my negative cable loose. I tighten it up and I have power again. But now it won't start unless I give it gas and keep it 1500 rpm or more. As soon as I let off the gas it dies. If I give it steady throttle to keep it running it still has a vibration/slight hesitation. It's like vvvvvvvvvvvvbbbbvvvvvvvvvvvvvbbbbvvvvvvvvvvvvvvvbb bbvvvvvvv....If that makes sense. Any Ideas? Sorry if this does not make sense. Please ask questions and I will try and explain better.

changed your plugs lately? do you have a service light? did any of your spark plug boots get. urned by the exhaust or could possibly come loose and your not getting direct contact? um....have you noticed any clanking/weird noise from the motor lately like a loose timing chain/broken valve springs?
if you try to hold it steady at 1000rpm does it die?

No noises or anything unusual. I checked all the spark plug wires and none of them were loose or burnt. I haven't changed the spark plugs in about 20,000. I replaced the timing chain with a ls2 when I did my cam change at the same time as the spark plugs. If I give it steady gas it will run at 1000 1500 and on up. But as soon as I let off it dies immediatly. One of the guys at work said he thought it was a vacuum leak but I don't know. Even at steady throttle it does not sound right. Every three seconds at a constant rpm the truck will shake or vibrate for about a quarter of a second but won't loose rpm. Then three seconds normal vibrate normal.
