Could be a u joint or the velocity joint on the front driveshaft.

The o ring is not needed unless your using a bolt on yolk the new trans yolk will be sealed on the end instead of having a bolt hole.

Just another benefit if being a machinist.... If you can't find a part you need you build It!
Take it to a machine shop and have them put it in a lathe and run a dial indicator on it to see if its bent. If its not bent I would be looking at the bearings.
Not saying it can't happen but cams are made of cast iron and they generally break instead bending.
