is the front axle engaging? There is still an electric actuater for the front end even on a manual shift t case. Maybe something is unplugged?
Don't balance the drive shaft.
You need to get your u joints straigtened out first. if it is moving side to side that is causing the vibe. no balance job will fix wrong u joints.
Its for a machine shop. they bolt them in place to simulate the cylinder head when they bore and hone.

If you can weld why not buy a set of spring perches and weld them onto the axle instead of using the flip kit?
If you do decide to do this buy an adjustable pinion snubber so you can raise it all the way up when you're crusing around. It will shake your teeth out otherwise.

Do you have a friend with a similar truck? You could try swapping clusters. it could be the stepper motor in there.
