With the smoke does it have a "metallic" weird taste? Do you have a test light or noir lights to make sure injectors are pulsing and not getting constant signal. Possible injector stuck open? When I was working out the base idle tune on my megasquirted turbo neon had similar issues but weird that you're truck was fine and now acting up. Leads me to mechanical. On you single low cylinder pull plug see if there is oil again also drop little bit of oil into cycle and do a compression check. If pressure comes up quite a bit prob a ring problem.
